How to Make an Appointment:

By phone: Please call (818) 342-5377.  This is the preferred method for making an appointment because we need certain demographic information prior to your first visit.  This includes your phone number, address, insurance type, and referring physician.  Certain insurance companies require pre-authorization for specialist consultation and we must have this before making the appointment.

By fax:  If you choose to contact us by fax (818) 996-9378, please be sure to include your 1) phone number, 2) fax number, 3) address, 4) name, 5) insurance type, and 6) referring physician.

By Email: You may email us at info@ArrhythmiaCenter.com for general questions, but we are currently not set up to arrange appointments by email.  This option is being evaluated for the future.

Cancellation policy:

Please give us and other patients the courtesy of at least 24 hour notice for cancellation so other patients may utilize that time slot for their appointments.
